MVP Web Platform for Configurable Questionnaires & Automated Reports

We are looking for an experienced web developer or small team to build an MVP web application that delivers multi-section questionnaires, applies rule-based scoring logic, and generates automated insight reports.

This is a non-clinical, non-diagnostic platform.
All question content, scoring logic, and report text will be provided. Scope of Work

1. User Access

Admin / practitioner login

Ability to create participant links (no participant login required)

2. Questionnaires

One main questionnaire

Four short supplementary questionnaires

Multiple-choice questions (4 options per question)

Questions and wording must be editable without code changes

3. Scoring Engine

Deterministic, rule-based scoring (no AI)

Each answer maps to predefined scoring categories

Scoring rules stored in configuration (JSON / database)

Support for basic tie-break logic

4. Reporting

Automatically generate one integrated report per participant

HTML report view

Downloadable PDF version

Clean, minimal presentation

5. Data Handling

Consent checkbox

Ability to delete participant data

Basic GDPR-aware handling
